Liquid Personnel is recruiting an experienced Occupational Therapist for its client’s Hospital Discharge Team based in Trowbridge, Wiltshire.

This is a full-time agency role offering up to Β£36 per hour to support Pathway 1 hospital discharges and promote safe and effective transitions from hospital to home.

You will conduct OT assessments, develop person-centred support plans, and work with multidisciplinary teams to ensure timely discharges and safe home return.
