Responsibilities:
- Supporting transitions planning - working with young people (14–25) to develop person‑centred transition plans that promote independence, wellbeing and long‑term outcomes.
- Completing Care Act assessments - assessing eligible needs as young people move into adulthood, creating strengths‑based support plans and coordinating appropriate adult services.
- Multi‑agency coordination - collaborating with education, health, SEND teams and families to ensure smooth transitions and consistent support across children’s and adults’ services.
