Liquid Personnel is currently recruiting for an experienced Hospital Social Worker to join our client's busy Hospital Discharge Team in Bury.

This is an excellent opportunity for a skilled practitioner with previous hospital social work experience to support adults with complex needs and play a key role in facilitating safe, effective, and timely hospital discharges.

What will your responsibilities be?

  • Manage a hospital-based social work caseload.
  • Complete assessments and support planning to facilitate safe discharges.
  • Work closely with NHS professionals, patients, families, and partner agencies.
  • Coordinate care packages and community support to prevent delayed discharges.
  • Undertake safeguarding responsibilities and risk assessments where required.
  • Maintain accurate and timely case records.
  • Provide cross-site cover across multiple hospital locations as required.
  • Participate in weekend working on a rota basis.

Qualifications & Experience

  • To be successful in this role you must have,
  • Qualified Social Worker (BA, MA, Dip SW, CQSW or equivalent).
  • Registered with Social Work England.
  • Previoushospital social work experienceis essential.
  • Experience supportinghospital discharge planning.
  • Strong assessment, communication, and multi-agency working skills.
  • Ability to work flexibly across hospital sites and weekends.

Compliance Requirements

  • Minimum of3 years' references.
  • Full employment history with any gaps explained.
  • Relevant qualifications in line with the role requirements.
