Our client is recruiting for an Occupational Therapist to join the team at Craven Vale, a vital 16‑bed reablement service that plays a key role in supporting hospital discharges and preventing unnecessary hospital admissions across Brighton. Role Overview
As the Occupational Therapist within the service, you will be central to the delivery of high‑quality reablement interventions, working with individuals transitioning from hospital and those requiring community‑based support to avoid admission. Your assessments and interventions will help maximise independence, reduce long‑term care needs, and facilitate safe and effective discharge planning.
Responsibilities
- Undertake Occupational Therapy assessments within a reablement setting.
- Develop and implement person‑centred intervention plans.
- Assess functional ability and identify opportunities to maximise independence.
- Recommend and arrange appropriate equipment and adaptations.
- Support timely and effective hospital discharges.
- Work collaboratively with multidisciplinary teams, care staff, and healthcare professionals.
- Contribute to preventing avoidable hospital admissions through proactive assessment and intervention.
- Maintain accurate and timely clinical records.
