Liquid Personnel is recruiting an experienced Social Worker in Derby to support adults with care needs within the Adult Social Care Community Team.

You will deliver person-centred social work interventions to promote independence and wellbeing.

You will undertake Care Act assessments, safeguarding enquiries, and Mental Capacity Assessments, working with service users, carers, families, and health partners to ensure timely, high-quality support and effective case management.
