Liquid Personnel is seeking an experienced Social Worker to join the Adult Services Community Team in North Somerset.

You will support adults, carers and families, using strengths-based, person-centred practice to promote independence, wellbeing and quality of life.

You will manage a varied caseload, undertake Care Act assessments and reviews, coordinate services with health and voluntary partners, and contribute to safeguarding and discharge planning while maintaining accurate case records.
