Liquid Personnel is recruiting a Social Worker for its client's All Age Disability Team at Wrexham.
What will your responsibilities be?
- Holistic Disability Assessment — Complete strengths‑based assessments and reviews for children, young people, and adults with lifelong disabilities, ensuring Care Act and Children Act duties are met and support is tailored to individual needs across the lifespan.
- Safeguarding & Complex Needs — Manage safeguarding concerns, undertake Section 42 enquiries where required, apply the Mental Capacity Act, and coordinate risk‑management plans for individuals with complex physical, learning, or developmental disabilities.
- Integrated Support Planning — Develop outcome‑focused support plans and coordinate multi‑agency involvement with health, education, SEND, therapy services, and community providers to ensure continuity of care and smooth transitions at key life stages.

Qualifications & Experience:
- To be successful in this role you must have:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
