Liquid Personnel is seeking an experienced and skilled Adult Social Worker to join their Ageing Well Team based in Wolverhampton. This role focuses on supporting older adults to maintain independence, improve wellbeing, and access appropriate care and support within the community. The successful candidate will be a confident practitioner with strong legal literacy, a person-centred approach, and experience working with older adults, including those with frailty, dementia, and complex needs.
Responsibilities:
- Completing statutory assessments including Care Act assessments, Mental Capacity Act (MCA) assessments, Best Interest decisions, and safeguarding enquiries.
- Developing strengths-based care and support plans that promote independence, wellbeing, and prevent or delay the need for long-term care.
- Supporting older adults with a range of needs including frailty, dementia, physical disabilities, and long-term health conditions.
- Undertaking adult safeguarding duties, identifying risks, investigating concerns, and working with partners to reduce harm.
- Managing a varied caseload within the community, ensuring timely interventions and compliance with statutory duties.
- Working collaboratively within a multi-disciplinary team, including health professionals, community services, voluntary organisations, and care providers to deliver integrated care.
- Supporting hospital discharges and working to prevent unnecessary admissions where appropriate.
Qualifications and Experience:
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
- Registration: Must be registered with Social Work England.
- Experience: Minimum of 5 years post-qualified experience (including ASYE), with a strong background in adult social care, ideally with older adults.
- Legislation Knowledge: Excellent knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding procedures, and best practice in ageing well services.
- Skills: Strong assessment, analytical, and communication skills, with the ability to build effective relationships and deliver person-centred outcomes.
- Driving Licence: Full UK driving licence and access to a vehicle is preferred due to community-based work.
