Liquid Personnel are currently recruiting for an experienced Occupational Therapist to join an Integrated Mental Health Team within Adult Social Care in Wokingham.
This is a fantastic opportunity for an OT with experience in mental health and social care who is passionate about supporting adults to live safely and independently within their community.
The role focuses on completing specialist assessments, care planning and interventions for individuals with significant mental health needs, helping them maximise their strengths, improve wellbeing and achieve meaningful outcomes.
You will work closely with individuals, carers, families and a wide range of professionals to provide person-centred support while promoting independence and positive risk-taking.
What will your responsibilities be?
- Complete strengths-based Occupational Therapy assessments for adults with mental health needs
- Undertake complex assessments, reviews and care/support planning
- Support individuals to identify goals and achieve greater independence
• Complete functional assessments covering areas such as
- Activities of daily living
- Meal preparation
- Shopping
- Money management
- Communication
- Motivation and engagement
- Meaningful activities
- Employment/training readiness
- Complete risk assessments and develop risk management plans
- Support safeguarding processes where required
- Work collaboratively with carers, families and partner agencies
- Provide advice around equipment, adaptations and assistive technology
- Liaise with OT colleagues regarding manual handling, equipment and housing adaptations
- Maintain accurate records using electronic case recording systems

Qualifications & Experience
- HCPC registration as an Occupational Therapist
- Experience working within mental health settings (including placement experience)
- Experience working in health or social care environments
- Strong assessment and clinical reasoning skills
- Experience managing risk and complex needs
- Ability to work autonomously and manage competing priorities
- Excellent communication skills with service users, families and professionals
- Full UK driving licence and access to a vehicle
Experience with
