Liquid Personnel is seeking an experienced Occupational Therapist to support the delivery of Hospital Discharge / Home First at Trowbridge. This role has become available due to increased service demand following the successful implementation of the Home First contract. The Occupational Therapist will work in partnership with acute hospital teams to support safe and timely discharges into the Reablement Service. The ideal candidate will have strong experience working in fast‑paced hospital environments and be confident managing high referral volumes and complex cases.
Responsibilities:
- Work collaboratively with staff from Great Western Hospital (GWH) and Royal United Hospital (RUH) to support discharge planning.
- Complete timely functional assessments to support safe transitions from hospital into the community.
- Provide clinical recommendations to enable safe discharge into the Reablement Service.
- Liaise with multidisciplinary teams, families, and partner services to promote independence and reduce delays.
- Record clear, accurate clinical notes and contribute to discharge planning meetings.
- Manage a varied and often fast-paced caseload.
Qualifications & Experience:
- Degree / Diploma in Occupational Therapy (essential).
- Experience in hospital discharge, Home First, or reablement pathways.
- Confidence working in a busy, fast-paced environment.
- Strong clinical reasoning and assessment skills.
- Ability to work effectively with acute hospital teams (GWH & RUH).
- Excellent communication skills and ability to manage competing priorities.
