Liquid Personnel is recruiting Social Workers for its client's Hospital Discharge Team at Southampton. Our client is looking for Hospital Social Workers to support and facilitate patient discharges from the hospital setting including University Hospital Southampton, Western Community Hospital and other local surrounding hospitals as and when needed. The main base for the role is at University Hospital Southampton supporting discharges from the acute setting. There is a requirement that staff will be available to come into the hospital for at least 2 days a week to support duty and in-reach visits.
The Team works in a strengths-based way supporting the value of Home First in relation to discharge planning and works alongside a multidisciplinary team (MDT) within the hospital setting to complete robust holistic assessments and support plans. The team also completes community reviews post-discharge to right-size care provision and ensure that this remains suitable outside of the acute setting.
Responsibilities:
- The hospital discharge team is responsible for assessing clients against care act eligibility and also works within the mental capacity act to make best interest decisions where needed.
- The team also completes safeguarding enquiries under section 42 of the Care Act alongside planning for discharge and within the community alongside the review process.
- The post is to support the team in a time of increased pressure and manage the current workload with extra staffing capacity.
Qualifications & Experience:
- Professional Qualification: A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).
- Registration: Must be registered with Social Work England, the regulatory body for social workers in England.
- Experience: 2+ years post-qualified experience as an adult social worker in a statutory setting.
- Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.
- Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
- Driving Licence: A full UK driving licence and access to a car are often required for client visits.
