Liquid Personnel is recruiting an experienced Social Worker for its client’s Adults Learning Disabilities Team based in Somerset. You will support adults with learning disabilities through person-centred assessments, care planning, and safeguarding, promoting independence and positive outcomes. Working as part of a multi-disciplinary team, you will collaborate with partner agencies to deliver effective, strengths-based interventions in line with statutory duties.
What will your responsibilities be?
- Undertake Care Act assessments, reviews and risk assessments for adults with learning disabilities and their carers
- Manage a caseload of varying complexity, responding to referrals and prioritising need effectively
- Develop and review person-centred care and support plans, promoting independence and wellbeing
- Carry out safeguarding enquiries and manage risk in line with statutory duties
- Support individuals in crisis, applying appropriate legislation to achieve timely outcomes
- Apply the Mental Capacity Act, including Best Interests decision-making where required
- Assess carers’ needs and implement appropriate support plans
- Prepare clear, accurate reports and maintain up-to-date case records
- Work collaboratively with families, advocates and multi-agency partners
- Facilitate and coordinate care packages with providers across statutory and independent sectors
- Participate in team duty rota and contribute to service delivery
Qualifications & Experience
- Degree in Social Work (BA, BSc, or MA) or an equivalent recognised qualification
- Registered with Social Work England
- Relevant experience within Adult Social Care, ideally within a Learning Disabilities setting
- Experience managing complex cases, including safeguarding and risk
- Experience working within multi-disciplinary teams and with partner agencies
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act
- Strong assessment, analytical, and decision-making skills
- Excellent communication and report-writing ability
- Ability to manage workload and competing priorities
- Competent IT skills for case recording and data management
- Ability to work independently and as part of a team
- Full UK driving licence and access to a vehicle (essential due to Somerset’s rural nature)
