Rate: £198 per day

Liquid Personnel is currently recruiting for a Social Worker to join an Adults LD team based in Stockton on Tees.

Responsibilities:

  • Support adults with learning disabilities, including young people transitioning into Adult Social Care.
  • Conduct Care Act assessments, develop person-centred support plans, and review care packages.
  • Apply the Mental Capacity Act, conduct capacity assessments and Court of Protection work.
  • Collaborate with health colleagues to support those detained under the Mental Health Act and assess eligibility for Continuing Health Care (CHC) funding.
  • Identify and address safeguarding concerns, including abuse, neglect, and self-neglect.

Ideal candidates will have:

  • Experience in Learning Disability teams.
  • Care Act assessments experience.
  • Mental Capacity Act work experience.
  • Strong written and verbal skills.
