Liquid Personnel is currently recruiting for a Social Worker to join an Adults LD team based in Stockton on Tees.
Responsibilities:
- Support adults with learning disabilities, including young people transitioning into Adult Social Care.
- Conduct Care Act assessments, develop person-centred support plans, and review care packages.
- Apply the Mental Capacity Act, conduct capacity assessments and Court of Protection work.
- Collaborate with health colleagues to support those detained under the Mental Health Act and assess eligibility for Continuing Health Care (CHC) funding.
- Identify and address safeguarding concerns, including abuse, neglect, and self-neglect.
Ideal candidates will have:
- Experience in Learning Disability teams.
- Care Act assessments experience.
- Mental Capacity Act work experience.
- Strong written and verbal skills.
