Responsibilities:
- Provide comprehensive, effective social work services for children and young people with disabilities, in line with legislation and departmental policies.
- Undertake strength-based assessments and develop outcome-focused support plans.
- Deliver interventions to safeguard children and young people, promote independence, and encourage assistive technology use.
- Support seamless transition for children and young people with disabilities from childhood through adulthood.
