Liquid Personnel is currently recruiting for an experienced Qualified Social Worker to join the South Community Team within Adult Social Care on a 3‑month contract with South Gloucestershire. This is a key frontline position within a locality Adult Social Care team. You will undertake social work assessments, provide advice and guidance, and support individuals to achieve positive outcomes. You will work as part of a multi-disciplinary team, collaborating closely with colleagues across health, social care, and the voluntary sector to ensure effective communication and the delivery of best practice.
What will your responsibilities be?
- Undertake assessments of need and develop person-centred care plans
- Review and monitor care plans, responding proactively to changes in needs
- Support and evaluate reablement programmes to promote independence
- Manage your own caseload, ensuring timely and effective interventions
- Carry out risk assessments and safeguarding work, including self-neglect cases
- Work in partnership with external agencies to coordinate support and services
- Promote awareness of care issues and ensure safe, effective practice

Qualifications & Experience:
To be successful in this role, you must have:
- Social Work England registration
- Valid DBS
- Strong knowledge of Deprivation of Liberty Safeguards (DoLS)
- Experience managing safeguarding cases and self-neglect situations
