Job Description:
Liquid Personnel is recruiting a Social Worker for it client’s Mental Health Hospital Discharge Team at Stockport.
What will your responsibilities be?
- You will be part of a all age adult mental health social work team but will focus mainly on completing proportionate care act assessments and support planning to enable people to be promptly discharged from hospital when they are detained under s3 of the mental health act.
- You will work collaboratively with our Pennine Care colleagues and have a crucial part to play in improving mental health services and mental health outcomes for people leaving hospital and their carers.
- You will support older and working age adults who also have a functional or organic mental health diagnosis to be discharged from hospital safely and in a timely manner.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
