Rate: £27.57 per hour
We are looking for dedicated Adult Social Workers to join our Hospital Team in Hammersmith and Fulham, offering a pay rate of £27.57 per hour.
Responsibilities:
- Conduct comprehensive assessments of patients’ social, emotional, and environmental needs.
- Develop and implement discharge plans in collaboration with patients, families, and healthcare teams.
- Manage a caseload of patients, ensuring effective and timely interventions.
- Monitor and review care plans to adapt to patients’ changing needs.

Qualifications and Experiences:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Degree level or equivalent in Social Work
- Post-qualified experience
