Our client in Leicestershire is seeking an experienced Adult Social Worker to join their Cognitive and Physical Disabilities Team. This role involves working with adults who have complex needs arising from physical disabilities, cognitive impairments, and long-term conditions. The successful candidate will be confident in managing a varied caseload, applying a strengths-based approach, and working within a multi-disciplinary environment to promote independence, wellbeing, and person-centred outcomes. This position requires a practitioner with strong legal knowledge, excellent assessment skills, and the ability to work proactively within a fast-paced adult social care setting.

What will your responsibilities be?

  • Undertaking comprehensive Care Act assessments, reviews, and reassessments for adults with physical and cognitive disabilities.
  • Completing Mental Capacity Assessments, facilitating Best Interest decisions, and contributing to DoLS and community-based deprivation of liberty work where required.
  • Developing and implementing person-centred care and support plans that promote independence, choice, and control, with a focus on strengths-based practice.
  • Managing a caseload involving individuals with physical disabilities, learning disabilities, acquired brain injuries, and other cognitive impairments, often presenting with complex needs.
  • Leading and contributing to adult safeguarding enquiries, ensuring appropriate risk management and safeguarding measures are in place.
  • Working collaboratively with health professionals, occupational therapists, community services, and care providers to deliver coordinated, holistic support.
  • Supporting hospital discharge pathways and community-based transitions, ensuring continuity of care and timely interventions.
  • Maintaining accurate, timely case recordings and ensuring compliance with statutory responsibilities and local policies.

Qualifications & Experience:

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Minimum of 5 years post-qualified experience, including completion of ASYE. Experience working with adults with physical and/or cognitive disabilities is essential.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and relevant case law.
  • Skills: High-level assessment, analytical, and communication skills, with the ability to manage complex cases and work effectively within a multi-agency framework.
  • Driving Licence: Full UK driving licence and access to a vehicle is typically required for community-based work.
