We are looking for a Social Worker to support young people and adults as they move from children's services into adult care. This is a crucial role in ensuring individuals experience a smooth and well-supported transition, enabling them to achieve independence, stability, and the best possible outcomes.

  • Hold a social work degree.
  • Right to work in the UK and hold clean UK driving license.
  • Registered with SWE.
  • Post qualified experience within children and family services.
