Job Title: Social Worker – Hospital Discharge team Location: Blackpool Council Rate : £***** per hour An exciting job role has recently become available at the Blackpool Council.
They are looking for a new member for their Adult\'s Hospital Discharge Team.
What will your responsibilities be?
In this role, you will undertake assessments and reviews, and where appropriate, commission services in line with departmental policies, procedures, and statutory guidance such as The Care Act *****).
You will understand and actively participate in the department\'s safeguarding adults\' procedures, including identification, investigation, and taking the role of Safeguarding Lead at H2.
You will also work closely with other teams, including undertaking joint assessments with providers, carers, and external agencies to deliver outcome-focused services to individuals and their families.
You will fully comply with departmental policies and procedures and the Health and Care Professionals Council\'s Code of Conduct for Social Workers, including the capability framework.
You will also comply with the requirements of the Care Act *****) by empowering individuals to make informed choices.
You will be required to maintain up-to-date knowledge of current issues and developments relating to social care and the wider health and social care agenda.
