Our client in Worcestershire is looking for an experienced Social Worker to join their Area Team for Adult Social Care.
Responsibilities:
- Conduct thorough social care assessments to identify the needs of adults with physical or mental impairments, frailty, and mental health concerns related to old age.
- Develop and implement personalised care plans.
- Provide crisis intervention support and manage a caseload effectively.
- Collaborate with multi-disciplinary teams to contribute to assessments that improve client outcomes while ensuring safeguarding measures are in place to protect vulnerable adults.
- Work closely with reablement services to maximise independence following hospital discharge or within a community setting, ensuring compliance with all relevant legislation, policies, and procedures.

Qualifications and Experience:
- Hold a recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
- Registered with Social Work England.
- Significant experience delivering social work services to vulnerable adults, including those with complex needs.
- Strong understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.
- Excellent assessment and analysis skills and ability to work effectively within a multi-disciplinary team.
- A full UK driving licence and access to a car are often required for client visits.
