Liquid Personnel is currently recruiting for an Adult Social Worker to join a Learning Disability team based in Bolton.

Responsibilities:

  • Conduct Care Act assessments, strengths-based support planning, and carers assessments.
  • Complete capacity assessments, Best Interest decisions, and Continuing Healthcare assessments.
  • Work collaboratively with multidisciplinary teams and lead safeguarding investigations.
  • Support individuals in transitioning to alternative accommodation, including resettlement from out-of-area placements.
  • Prepare reports for tribunals, hospital discharges, Community Treatment Reviews, and Court of Protection applications.

Qualifications and Experience:

  • SWE registration
