Liquid Personnel is seeking an experienced adult social worker to join their esteemed client. The position is for a Social Worker to become a part of the Adult Disabilities Team. This role involves working within a small, dynamic team dedicated to supporting adults with disabilities. The primary focus is to ensure that these individuals receive the necessary care and support to lead fulfilling lives.
Responsibilities:
- Manage a diverse caseload, prioritising cases based on urgency and need, and ensuring timely interventions and support for adults with disabilities.
- Identify and respond to safeguarding concerns, conducting investigations and implementing protection plans to ensure the safety of vulnerable adults with disabilities.
- Conduct assessments to determine the mental capacity of adults with disabilities, ensuring decisions are made in their best interests.
- Collaborate with a range of professionals, including healthcare providers, therapists, and community organisations, to provide holistic support to adults with disabilities.
- Advocate for the rights and needs of adults with disabilities, ensuring their voices are heard and their preferences are respected in care planning and decision-making processes.
- Provide immediate support and intervention in crisis situations, working to stabilise and support adults with disabilities and their families.
- Maintain accurate and up-to-date records of assessments, care plans, and interventions, ensuring compliance with legal and organisational standards.
- Conduct comprehensive social care assessments to identify the needs and strengths of adults with disabilities, ensuring their well-being and safety.
- Develop, implement, and review personalised care plans in collaboration with adults with disabilities, their families, and other professionals to meet their individual needs.

Qualifications and Experience:
- A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).
- Must be registered with Social Care Wales, the regulatory body for social workers in Wales.
- Significant experience in delivering social work services to vulnerable adults, including those with disabilities and complex needs.
- Thorough understanding of relevant legislation, such as the Social Services and Well-being (Wales) Act 2014 and the Mental Capacity Act 2005.
- Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
- A full UK driving licence and access to a car are often required for client visits.
