Liquid Personnel is recruiting Senior Social Work Practitioner for its clients Preparing for Adulthood Team at Shropshire. Our client, based in Shropshire, is seeking an experienced and committed Senior Social Work Practitioner to join their Preparing for Adulthood Team. This role focuses on supporting young adults as they transition into adulthood, promoting independence, wellbeing, and positive long-term outcomes.
You will be part of a collaborative, multi-disciplinary service working with individuals whose disabilities significantly impact their daily lives, ensuring high-quality, person-centred care and support.
What will your responsibilities be?
- Work in partnership with young people, adults, and their families/carers to assess needs and develop outcome-focused support plans.
- Undertake statutory assessments, safeguarding activity, and care planning, ensuring wellbeing and safety is prioritised.
- Assess risk and deliver interventions to improve quality of life and long-term independence.
- Coordinate multi-agency services, working closely with health, education, housing, and partner organisations.
- Lead on preparation for adulthood and transition planning, enabling continuity of care and independence.
- Maintain accurate, up-to-date case records in line with statutory and data protection requirements.
- Engage in supervision, training, and continuous professional development.
Qualifications & Experience:
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
- Registration: Must be registered with Social Work England.
- Experience: Experience within a statutory setting working with young people transitioning to adulthood and/or adults, including safeguarding.
- Knowledge: Strong understanding of relevant legislation relating to adult social care, safeguarding, and transition services.
- Skills: Strong assessment, analytical, and care planning ability; ability to manage complex caseloads and competing priorities; excellent communication and multi-agency working skills.
- IT Skills: Experience using electronic case management systems and maintaining accurate records.
