In this role, you will conduct Occupational Therapy assessments and interventions. You will work as part of the Reablement service, meeting statutory requirements. You will collaborate with the social work team for a holistic, strengths-based approach. You will manage hospital discharge processes and assess daily living activities. You will recommend and order appropriate equipment and home adaptations. You will maintain detailed records using MOSAIC. You will develop relationships with external partners and providers. You will complete full OT assessments and make recommendations for equipment and adaptations. You will work collaboratively with social work colleagues to assess clients. You will implement rehabilitation plans for service users in supported living. You will conduct assessments for Telecare, Taxicard, and Freedom passes. You will provide support for Disability Facilities Grants (DFG) and major adaptations. You will also manage a clinical caseload of generic adults meeting
OT eligibility criteria.
