Liquid Personnel is looking for experienced Occupational Therapist to support its client's Adult Social Care service team in Rutland, delivering high-quality interventions to enable individuals to remain independent in their own homes. This is a varied role, combining reablement, discharge support, and community-based interventions.
What will your responsibilities be?
- Complete holistic, strengths-based assessments and develop personalised reablement plans
- Support individuals on Discharge to Assess pathways, enabling safe return home
- Work with community patients to prevent hospital admissions
- Assess and provide equipment and adaptations (including major adaptations)
- Complete risk assessments, including complex moving & handling cases
- Promote and support use of assistive technology
- Work collaboratively with health and social care professionals
- Maintain accurate case records and monitor support plans
- Supervise OT Assistants and provide guidance to colleagues

Qualifications & Experience:
- Degree/Diploma in Occupational Therapy
- HCPC Registration
- Experience within Adult Social Care or Health settings
- Understanding of: Care Act 2014, Safeguarding procedures
- Strong assessment, risk management, and care planning skills
Key Skill:
- Ability to manage and prioritise workload effectively
- Strong communication and partnership working
- Confident in moving & handling assessments
- IT literate (case management systems, email, data input)
Working Pattern:
- Hybrid working: 2 days office-based, 1 day working from home, remaining time in the community
