Job Title: Social Worker – Transition Team

Location: Bristol

Rate: £32 per hour

Bristol is actively seeking a full-time Social Worker to join our dedicated Transitions Team.

Responsibilities:

• Support triage decision-making processes
• Produce care plans
• Work directly with individuals in Bristol transitioning to adulthood
• Ensure they receive necessary support and guidance during this critical period

Qualifications and Experience:

Must have Social Work England registration, be eligible to work in the UK, hold a full UK license, have post-qualified experience, and possess a degree or equivalent in Social Work.
