Liquid Personnel is recruiting for an experienced Occupational Therapist to join Wiltshire Council's Reablement Team, supporting adults following hospital discharge and helping them regain independence. This is a fantastic opportunity to work within a supportive team delivering person-centred assessments and reablement interventions across home, hospital, and community settings.
What will your responsibilities be?
- Complete Occupational Therapy assessments following hospital discharge
- Develop tailored reablement plans to maximise independence
- Provide advice on equipment, moving safely, and best practice
- Work collaboratively with colleagues to support decision-making and positive outcomes
- Carry out community visits while managing a varied caseload
Benefits:

Qualifications & Experience:
- Qualified Occupational Therapist
- Minimum 2 years' post-qualification experience
