Liquid Personnel is recruiting an experienced Occupational Therapist for its client’s Hospital Discharge Team based in Trowbridge, Wiltshire. This is a full-time agency role offering up to £36 per hour and supporting Pathway 1 hospital discharges to promote safe and effective transitions from hospital to home.
The Role
Wiltshire Council is seeking an experienced Occupational Therapist to join its fast-paced and supportive Hospital Discharge Team based at County Hall, Trowbridge. You will play a key role in facilitating timely hospital discharges, promoting independence, and ensuring individuals receive the right support to return home safely.

Key Responsibilities
- Complete occupational therapy assessments for individuals being discharged from hospital
- Undertake same-day discharge assessments where appropriate
- Develop person-centred support plans to maximise independence
- Recommend equipment, adaptations, and therapeutic interventions to support safe discharge
- Work collaboratively with multidisciplinary teams, hospital staff, and community services
- Maintain accurate, timely, and compliant case records
Essential Requirements
- Degree or Diploma in Occupational Therapy
- Current HCPC registration
- Strong assessment, communication, and clinical reasoning skills
- Ability to work effectively within a fast-paced multidisciplinary environment
Desirable Experience
- Previous experience within hospital discharge services
- Reablement or adult social care experience
- Knowledge of Pathway 1 discharge processes
