Job Description
Liquid Personnel is recruiting an Occupational Therapist to join the Discharge to Assess Project team in East Sussex. In this role, you’ll work closely with Adult Social Care Hospital Teams to support safe, timely hospital discharges and help people return home with the right support.
Responsibilities
- Complete holistic, strengths-based assessments for adults ready to leave hospital
- Coordinate equipment provision and home adaptations to promote independence
- Liaise with hospital teams, community services, and equipment providers
- Support service users and carers through the discharge process
- Promote safe, person-centred transitions from hospital to home
- Maintain accurate and timely documentation in line with professional standards

Qualifications & Experience
To be successful in this role you must have,
- Degree or Diploma in Occupational Therapy
- HCPC registration (essential)
- Experience working in hospital discharge, community, or social care settings
- Strong knowledge of equipment provision, home adaptations, and D2A pathways
- Excellent communication, organisational, and collaborative working skills
- Commitment to promoting independence and person-centred care
