Liquid Personnel is recruiting experienced Social Workers for its client to support a Hospital Discharge Service with a client based in Newport, working on-site within an acute hospital setting. This service plays a vital role in ensuring patients are supported to leave hospital safely and at the earliest opportunity. The team operates within a fast-paced, multi-disciplinary environment, working closely with healthcare professionals and partner agencies to deliver effective discharge planning and improve outcomes for individuals and their carers. The Home First approach is central to the service, offering an exciting opportunity for practitioners to influence both discharge and admission practices while enhancing the overall hospital experience for the local community.

What will your responsibilities be?

  • Support timely and safe hospital discharges, ensuring individuals transition into appropriate care settings.
  • Work closely with patients, families, and carers to assess needs and coordinate discharge plans.
  • Collaborate with multi-disciplinary teams, including health professionals and external agencies.
  • Arrange care packages, placements, and community-based support services.
  • Undertake assessments and care planning in line with legislation and best practice.
  • Promote independence through a Home First approach, ensuring least restrictive options are considered.
  • Maintain accurate and up-to-date records using electronic case management systems.

Qualifications and Experience:

  • Professional Qualification: MA, BA, or BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Care Wales (SCW).
  • Experience: Minimum of 2 years’ post-qualified experience (ASYE completed).
  • Skills: Strong assessment, communication, and organisational skills, with the ability to work effectively in a busy hospital environment. Experience within hospital discharge or multi-disciplinary settings is highly desirable.
  • IT Skills: Confident using electronic recording systems and maintaining high-quality documentation.
