What will your responsibilities be?
- Conduct comprehensive assessments of young people’s needs and create personalized transition plans.
- Manage a caseload of young adults, coordinating care across services and agencies, while advocating for their rights and ensuring access to appropriate support.
- Work closely with multidisciplinary teams—including healthcare professionals, educators, and local authorities—to deliver integrated care.
- Provide emotional and practical support to young people and their families during transitions, ensuring continuity and stability.
All responsibilities will be carried out in line with the Children and Families Act (CFA) and relevant safeguarding legislation.

Qualifications & Experience:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
