Key Responsibilities:
- Manage a caseload of customers with varying levels of complexity and risk
- Conduct specialist assessments in homes to determine eligibility and risk
- Recommend and arrange equipment and adaptations to promote safe, independent living
- Design and specify cost-effective home adaptations, including scale drawings
- Prescribe wheelchairs and accessories in line with NHS guidance
- Provide emotional support to customers and carers during major environmental changes
- Complete re-housing assessments and compile detailed reports
- Carry out moving and handling risk assessments and provide training to carers and professionals
What We’re Looking For:
- A qualified Occupational Therapist registered with HCPC
- Experience working with individuals with physical, mental, or learning disabilities
- Strong assessment and care planning skills
- Ability to work collaboratively with health, housing, and voluntary sector partners
