Key Responsibilities:

  • Manage a caseload of customers with varying levels of complexity and risk
  • Conduct specialist assessments in homes to determine eligibility and risk
  • Recommend and arrange equipment and adaptations to promote safe, independent living
  • Design and specify cost-effective home adaptations, including scale drawings
  • Prescribe wheelchairs and accessories in line with NHS guidance
  • Provide emotional support to customers and carers during major environmental changes
  • Complete re-housing assessments and compile detailed reports
  • Carry out moving and handling risk assessments and provide training to carers and professionals

What We’re Looking For:

  • A qualified Occupational Therapist registered with HCPC
  • Experience working with individuals with physical, mental, or learning disabilities
  • Strong assessment and care planning skills
  • Ability to work collaboratively with health, housing, and voluntary sector partners
