Liquid Personnel is looking for an Occupational Therapist to join its client's Recovery Hub Service, working within a fully integrated team focused on rehabilitation, independence, and safe discharge home team in Leeds. You’ll be part of a multi-disciplinary team including OTs, Physiotherapists, Nurses, and care staff, promoting a 24-hour therapeutic approach to recovery.
What will your responsibilities be?
- Carry out strengths-based Occupational Therapy assessments
- Develop and implement goal-led, outcome-focused rehabilitation plans
- Support individuals in achieving independence and returning home safely
- Lead on discharge planning back to the community
- Work alongside support staff to monitor and review progress
- Undertake moving & handling risk assessments and implement safe solutions
- Promote single-handed care principles where appropriate
- Collaborate with MDT colleagues across health and social care
- Maintain accurate and compliant case records

Qualifications & Experience:
To be successful in this role you must have:
- Degree/Diploma in Occupational Therapy
- HCPC Registration
- Strong experience in: Rehabilitation / Reablement, Moving & Handling assessments, Strengths-based practice
- Ability to manage a caseload and deliver outcome-focused care
Desirable:
- Experience within integrated recovery or discharge services
- Background in community or intermediate care settings
