About the Role:
Liquid Personnel is currently recruiting for registered and experienced Qualified Adult's Social Workers to join a Hospital Team based in Lambeth.
What will your responsibilities be?
To be successful in this role, you will support residents admitted to hospitals within and outside the borough. Your main duties include delivering strength-based preventative support, assessing needs, establishing support systems, and safeguarding patients. You will review care packages and consider placements. As a hospital social worker, you will refer individuals to reablement services to enhance their living skills and ensure timely discharge decisions. Collaboration with multidisciplinary colleagues, voluntary sector organisations, and wider agencies is essential. Adaptability, partnership commitment, and experience with adults with physical health disabilities and vulnerabilities are crucial. You will need a robust understanding of safeguarding procedures to excel in the role.

Qualifications & Experience:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving license. Additionally, a degree-level qualification or equivalent in Social Work and at least 2 years of post-qualified experience are essential.
