Liquid Personnel is seeking a dedicated Social Worker to join its client's Hospital Discharge Team in the London Borough of Richmond upon Thames. In this role, you will play a crucial part in ensuring safe, timely, and person-centred hospital discharges, helping adults return to their homes or community settings with the right care and support. This position promotes independence, wellbeing, and embraces a "Home First" approach.

What will your responsibilities be?

  • Coordinate and manage the discharge process for adults with diverse and complex needs.
  • Work within the Discharge to Assess (D2A) framework to facilitate timely transitions.
  • Undertake Care Act compliant assessments, including Mental Capacity Act assessments and Best Interest decisions.
  • Develop strengths-based, person-centred care plans that promote independence and choice.
  • Collaborate with health professionals, therapy teams, housing providers, and voluntary sector partners.
  • Identify and respond to safeguarding concerns in line with statutory guidance and local policy.
  • Maintain accurate records and ensure adherence to relevant legislation (Care Act, MCA, DoLS).
