Our client based in Derby is seeking an experienced Social Worker to join their Adult Social Care Community Team.

This is an excellent opportunity for a dedicated practitioner to support adults with care and support needs through the delivery of high-quality, person-centred social work interventions that promote independence, wellbeing, and positive outcomes.

The successful candidate will undertake a varied caseload of statutory social work responsibilities, including Care Act assessments, reviews, safeguarding enquiries, and Mental Capacity Assessments.

Working within a supportive community team, you will collaborate with service users, carers, families, health professionals, and partner agencies to ensure individuals receive the right support at the right time.

This role also includes participation in the team's duty rota and requires regular office presence to support service delivery and collaborative working.

What will your responsibilities be?

  • Undertaking Care Act 2014 assessments and reviews to identify eligible needs and support positive outcomes for adults with care and support needs.
  • Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
  • Managing a varied community-based caseload, ensuring statutory responsibilities are completed within required timescales.
  • Undertaking Adult Safeguarding enquiries and interventions in accordance with safeguarding legislation, policy, and best practice.
  • Completing Mental Capacity Assessments in line with the Mental Capacity Act 2005 and contributing to Best Interest decision-making where required.
  • Assessing and managing risk appropriately, balancing individual choice and control with safeguarding responsibilities.
  • Working collaboratively with service users, carers, health professionals, community services, and partner agencies to coordinate effective support.
  • Participating in the duty rota on a weekly basis, responding to urgent referrals and situations requiring immediate intervention.
  • Maintaining accurate, timely, and professional case records, assessments, reports, and support plans.
  • Promoting strengths-based practice and supporting individuals to access community resources and informal support networks wherever appropriate.
  • Attending the office on designated team days and duty days to support effective communication, service delivery, and multidisciplinary working.

Qualifications and Experience

  • To be successful in this role you must have;
  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake Care Act assessments, reviews, Mental Capacity Assessments, and support planning.
  • Safeguarding Experience: Experience managing safeguarding enquiries and implementing appropriate interventions.
  • Multi-Agency Working: Experience working collaboratively with health professionals, carers, families, and partner agencies.
  • Communication Skills: Excellent assessment, communication, report-writing, and decision-making skills.
  • Professional Practice: Commitment to person-centred, strengths-based practice and continuous professional development.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable.
