Liquid Personnel is recruiting Social Worker an experienced Adult Social Worker to join its client's Adult Social Care Locality Team at Derby.

This is an excellent opportunity for a motivated and skilled practitioner to support adults with a wide range of care and support needs through the completion of statutory assessments, reviews, and support planning.

The successful candidate will be confident in managing a varied caseload independently, balancing competing priorities whilst delivering person-centred, strengths-based interventions.

You will work closely with adults, carers, health professionals, and partner agencies to promote independence, wellbeing, and positive outcomes for individuals within the local community.

What will your responsibilities be?

  • Undertaking Care Act assessments to identify eligible care and support needs and develop appropriate interventions.
  • Completing reviews of existing care and support arrangements, ensuring services remain effective, proportionate, and outcome-focused.
  • Developing and implementing person-centred support plans that promote independence, wellbeing, and choice.
  • Managing a varied caseload involving adults with physical disabilities, learning disabilities, mental health needs, sensory impairments, and older people.
  • Working autonomously to prioritise workload effectively and respond to changing risks and demands.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making where required.
  • Identifying and responding to safeguarding concerns, working in line with statutory guidance and local safeguarding procedures.
  • Working collaboratively with service users, carers, health colleagues, care providers, and community organisations to achieve positive outcomes.
  • Promoting strengths-based practice and supporting individuals to access community resources and preventative services wherever possible.
  • Maintaining accurate and timely case records, assessments, reviews, and reports in line with departmental policies and professional standards.

Qualifications & Experience

  • To be successful in this role you must have,
  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care, including completing assessments, reviews, and support plans.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and associated statutory guidance.
  • Skills: Excellent assessment, communication, analytical, and report-writing skills with the ability to manage competing priorities effectively.
  • Multi-Agency Working: Proven experience working collaboratively with partner agencies and multidisciplinary professionals.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable for community-based visits
