Liquid Personnel is recruiting an Adult Social Worker to join its client’s Dementia and Care Quality team, based in Cardiff.
About the role:
Cardiff Council is looking for an experienced and committed Social Worker to join the Dementia and Care Quality Team. This is a fantastic opportunity to contribute to high quality care provision while supporting individuals living with dementia and those in care home placements. You will work closely with partners across health and social care to ensure safe, effective, and person centred outcomes for citizens and their families.
What will your responsibilities be?
- Carry out statutory reviews for citizens in care home placements
- Work collaboratively with Health partners to support changes in care home status, including Continuing Healthcare
- Act as a key contact for care home managers and support colleagues with unplanned reviews
- Contribute to provider performance processes, including care home closures, ensuring a structured and supportive approach for citizens and families
- Work alongside Contracts and Commissioning to maintain consistent processes and uphold best practice in care home and dementia care
- Promote best practice across the service and partner organisations in supporting individuals with dementia
- Support benchmarking and continuous development of knowledge across teams and allied services
- Assess placement suitability and manage cases effectively in partnership with service users, families, and providers
- Undertake Mental Capacity Assessments and Best Interest Decisions

Qualifications and Experience:
To be successful in this role you must have:
- Qualified Social Worker with current registration
- Experience working with adults, ideally within dementia or care home settings
- Strong understanding of the Mental Capacity Act and Best Interest processes
- Excellent communication and partnership working skills
- Ability to manage complex caseloads and contribute to service development
- Person centred, proactive, and solution focused approach
