Liquid Personnel is seeking an experienced Social Worker to join its clients Hospital Discharge Team, supporting citizens across discharge pathways.

What will your responsibilities be?

  • Assess and support citizens on discharge from hospital across three pathways
  • Complete post-discharge assessments and reviews
  • Undertake Care Act Assessments (CAA) and support planning
  • Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work
  • Lead on safeguarding enquiries and interventions
  • Provide duty cover as required
  • Attend Quality Assurance panels for funding provisions
  • Support Discharge to Assess (D2A) pathways

Requirements:

  • Proven experience within hospital discharge settings
  • Strong working knowledge of Care Act, MCA, safeguarding, and discharge processes
  • Ability to manage a fast-paced caseload and work under pressure
  • Excellent assessment and decision-making skills
  • Available to start immediately

Qualifications & Experience:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
