About the Role:

Join the Adult Learning Disability team to support adults and young people with learning disabilities by completing Care Act Assessments and reviews. What will your responsibilities be?

  • Support adults and young people with learning disabilities by completing Care Act Assessments and reviews.
  • Work collaboratively within a multi-disciplinary team, including Adult Social Care and NHS Health staff.
  • Manage safeguarding cases, assess risk and coordinate care management effectively.
  • Engage with service users, families and partner agencies to promote person-centred support.
  • Demonstrate strong communication, organisational skills and a commitment to improving outcomes for individuals with learning disabilities.

Qualifications & Experience:

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level and have at least 2 years of post-qualified experience.
