An exciting role has become available at the London Borough of Newham Council. They are looking for an experienced Adults Social Worker to join their Review team.
Responsibilities include:
- Conduct strengths-based, person-centred assessments to identify needs and strengths of adults in the community.
- Deliver exceptional social care, promoting choice and control.
- Engage in outcomes-focused conversations, building on individual and community strengths.
- Review care plans regularly to meet evolving needs and statutory requirements.
- Coordinate and deliver integrated care with health professionals, housing services, voluntary organizations, and other stakeholders.
Qualifications and Experience:
- Social Work England registration and eligibility to work in the UK.
- Full UK driving licence.
- Degree level qualification or equivalent in Social Work.
