Our client is seeking a dedicated and qualified Adult Social Worker to join their team. As an Adult Social Worker, you will be responsible for providing support and assistance to adults in need, ensuring their well-being and promoting their independence. You will work closely with other professionals to deliver high-quality care and support services.

Your responsibilities as a Social Worker:

  • Manage an adult caseload, performing assessments and care management tasks while participating in a general duty rota under the supervision of the Team Manager.
  • Conduct and coordinate assessments using Personal Outcome Planning tools in collaboration with service users, carers, and partner agencies.
  • Develop and implement care packages that meet the individual needs of adults and young people transitioning to Adult Services.
  • Oversee care management to ensure care packages remain effective and responsive to the changing needs of service users and their carers.
  • Support the Team Manager in delivering high-quality services and ensuring the efficient implementation of agreed care plans.
