Liquid Personnel is recruiting an Adult Social Worker to join its clientâs Transitions Team, based in Devon. As a Social Worker in our Adult Community Social Care team, youâll work with people aged 17+, supporting smooth transitions from childrenâs to adult services and working with adults who have a range of physical disabilities, learning disabilities and longâterm health conditions. Youâll be part of a friendly, experienced and integrated team where reflective practice, collaboration and good social work are genuinely valued.
What will your responsibilities be?
- Undertake Care Act assessments and reviews
- Support MCAâcompliant decisionâmaking
- Safeguard adults at risk of abuse or neglect
- Work in a strengthsâbased, personâcentred way
- Build strong partnerships with individuals, families, providers and health professionals to promote independence and safe living within the community
Qualifications & Experience:
- Professional Qualification: A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).
- Registration: Must be registered with Social Work England, the regulatory body for social workers in England.
- Experience: 2+ years post qualified experience as an adult social worker in a statutory setting.
- Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.
- Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
- Driving Licence: A full UK driving licence and access to a car are often required for client visits.
