Responsibilities:
- Evaluate the requirements of individual service users and their caregivers for social care services.
- Conduct crisis and risk evaluations.
- Create personalized support plans based on assessed needs and budgets.
- Promote the use of direct payments to help individuals manage their care independently, while guiding them towards necessary support.
- Identify and address the needs of vulnerable individuals at risk of harm, respecting those with decision-making capacity.
- Develop partnerships with various public and independent agencies to support collaborative working as outlined in “Working Together.”
