Liquid Personnel is recruiting an experienced Social Worker for its client's Older Adults Team at Waltham Forest. You’ll be working with individuals aged 65+ with a range of needs, including physical frailty, dementia, and long-term conditions.
You’ll undertake strengths-based assessments, develop and review care plans, and collaborate with health professionals and families to safeguard and support independence. Experience with Mosaic/Liquidlogic is preferred.
