Position Overview

The Case Manager at Lions Campus plays a pivotal role in our client’s recovery journey. This multifaceted role requires a compassionate professional who can work 1-1 with a client while ensuring cohesive coordination amongst a client\\\’s team. The Key Worker is a primary point of contact for clients and their families, guiding them through every step of their recovery process.

Responsibilities

  • Therapeutic Support: Conduct individual and group sessions tailored to clients’ needs, fostering a supportive environment for shared growth and recovery.
  • Provide psychoeducation to clients and families about addiction, mental health, and recovery.
  • Conduct assessments and develop cohesive support plans for clients, addressing their mental health and well-being needs.
  • Case Management: Develop, implement, and regularly review individualized support plans.
  • Manage a caseload of clients, ensuring timely and effective delivery of care.
  • Work collaboratively with a team of Recovery Workers to enhance client engagement in the structured programme.
  • Coordinate interdisciplinary care by liaising with medical professionals, therapists, and external service providers.
  • Facilitate partnerships with mental health, healthcare services, and other relevant agencies.
  • Administrative Duties: Maintain accurate and confidential client records in accordance with organisational policies and ethical standards.
  • Communicate treatment plans and progress reports with sensitivity, adhering to ethical requirements regarding confidentiality.
  • Stay updated on advancements in addiction and mental health treatment to enhance care quality.

Qualifications

  • Bachelor’s or Master’s degree in Counselling, Psychotherapy, Psychology, Social Work, or a related field.

  • Experience working with adolescents
