As part of the NHS England Additional Roles Reimbursement Scheme (ARRS), we are seeking a skilled and motivated Advanced Nurse Practitioner (ANP) to join our Primary Care Network (PCN) team.

The Advanced Nurse Practitioner (ANP) will be able to assess and treat varied clinical cases, having advanced skills to assess the patient with a differentiated or undifferentiated clinical diagnosis.

Advanced communication, problem-solving and decision-making skills will be used to initially assess and provide on-going patient care.

Working within local and nationally agreed protocols and guidelines, the ANP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of patients.

  • The role is for Monday, Thursday and Friday, Clinical Assessment and Care
  • Provide expert clinical assessment, diagnosis, and treatment for a wide range of acute and chronic conditions, within the scope of practice.
  • Conduct comprehensive health assessments for patients with complex care needs and those with multiple long-term conditions.
  • Prescribe medications and interventions in accordance with clinical guidelines and scope of practice.
  • Undertake home visits as needed to assess and support patients in their own environment.

  • Develop, implement, and evaluate patient management plans in collaboration with patients, their families, and the multidisciplinary team.
  • Deliver safe, evidence-based, and patient-centred care, ensuring that the individuals needs are met.
  • Provide follow-up care and support, monitoring progress and adapting treatment plans where necessary.

We work collaboratively across our member practices and partner organisations to provide accessible healthcare services and reduce health inequalities.

The successful applicant will be based at The Limes Medical Centre and will also support patients registered at Chapel Street Surgery, offering the opportunity to work across practices and gain experience with a diverse patient population.

You will be part of a supportive multidisciplinary team, working closely with both PCN and practice colleagues.

Our wider PCN team includes Clinical Pharmacists, First Contact Practitioners, Care Coordinators, Social Prescribers, Mental Health Practitioners and Health and Wellbeing Coaches, alongside operational and administrative staff.

At practice level, you will work alongside GPs, Practice Nurses, Healthcare Assistants, Reception and Care Navigation teams, and Practice Management staff.

Multidisciplinary working is central to our approach, helping us deliver joined-up, high-quality care while supporting learning, development and career progression.
