We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to join our Primary Care Network (PCN). The ACP will provide high‑quality, patient‑centred care, working autonomously to assess, diagnose, treat and manage patients with a wide range of acute, chronic and complex health conditions. The successful candidate will undertake advanced clinical assessments, develop and implement treatment plans, prescribe medication where appropriate, and order and interpret diagnostic investigations. They will work closely with GPs and the wider multidisciplinary team to ensure safe, effective and integrated care for patients.
Key Responsibilities
- Provide follow‑up care, support patients to manage their health, and promote health education and wellbeing.
- Contribute to the continuous improvement of services.
- Support clinical governance, participate in audits and quality improvement initiatives, maintain accurate patient records and ensure compliance with professional, organisational and regulatory standards.
- Act as a clinical resource within the team, sharing expertise, supporting colleagues and contributing to service development.
Additional Context
As part of the NHS England Additional Roles Reimbursement Scheme (ARRS), the ACP will assess and treat varied clinical cases with advanced skills, applying independent judgement to investigate, diagnose, plan, implement and evaluate the clinical care and management of patients within local and nationally agreed protocols and guidelines.
