About the Role

We are currently recruiting for an experienced Agency Social Worker to join the Team for Older People within Wrexham County Borough Council. Based at Crown Buildings, Wrexham, this is a varied hybrid role combining office-based working, community and hospital visits, and home-based working. You will manage a caseload of older adults, working collaboratively with citizens, families, carers and partner agencies to promote independence, maximise wellbeing and achieve positive outcomes. You will also participate in the team's duty rota, supporting the management of new referrals, urgent enquiries and priority work.

Key Responsibilities

  • Manage a caseload of older adults, providing high-quality strengths-based social work.
  • Undertake comprehensive needs-led assessments and develop appropriate care and support plans.
  • Review care and support arrangements to ensure they continue to meet the needs and desired outcomes of citizens.
  • Complete Mental Capacity Assessments and Best Interest decisions in accordance with legislation and professional standards.
  • Respond appropriately to adult safeguarding concerns and manage associated risks.
  • Work collaboratively with citizens, families, carers and professionals across health, social care and voluntary sector services.
  • Undertake visits in community and hospital settings as required.
  • Participate in the team's duty rota, responding to new referrals, urgent enquiries and priority cases.
  • Complete high-quality case recordings, assessments, reports and professional documentation.
  • Make informed professional decisions regarding risk, care and support.
  • Promote independence, choice, control and positive outcomes for older people.
  • Develop effective partnership working across multi-agency services.
  • Travel throughout Wrexham County as required.

About You

We are looking for a qualified and experienced Social Worker with a strong background in adult social care and experience working with older people. You will have:

  • A recognised Social Work qualification.
  • Current registration with Social Care Wales.
  • Experience in adult social work assessment, care planning and reviews.
  • Proven experience working with older people.
  • Experience undertaking Mental Capacity Assessments and Best Interest decisions.
  • Experience managing and responding to adult safeguarding concerns.
  • Strong assessment, analytical and professional decision-making skills.
  • Excellent report-writing and case management abilities.
  • Strong communication and relationship-building skills.
  • The ability to work effectively with citizens, families, carers and multi-agency professionals.
  • The ability to travel throughout Wrexham County as required.

Please note: Registration with Social Work England alone is not sufficient for this position. Candidates must hold current Social Care Wales registration.

Desirable Experience

The following experience would be advantageous:

  • Completion of Decision Support Tools (DSTs).
  • Experience of Court of Protection work.
  • Managing complex adult social care cases.
  • Experience working across hospital and community settings.
  • Previous local authority adult social care experience.

Important Requirement

Applicants must live within 45 minutes of Wrexham and be able to travel to Crown Buildings and locations throughout the county as required.
